The Director of Case Management is responsible for leading the Care Management teams in providing early planning and intervention to facilitate appropriate level of care, maintain patient care quality, and decrease avoidable days. Incumbent will lead the development and implementation of appropriate care management and discharge plans, and supports and facilitates Care Managers role in responsibility and accountability for all care management team efforts, including social work and utilization activities.
MUST HAVE REQUIREMENTS:
BSN
CCM Certification
5 years experience in hospital clinical role
5 years utilization management, case management, discharge planning, cost/quality management program
